Privately Funded

Being able to remain living at home is one of the most important priorities for many people, both elderly and those with a disability. We are specialists in aged, disability and palliative care for clients of any age, with private home care as a practical option for long term or short-term care.

With private home care there are no waiting lists, government assessments or restrictions, so you can access these services at any age, for any length of time. Please call us to discuss your individual needs.

Insurance Funded

Whether you have been severely injured in the workplace, or in a motor vehicle accident, or otherwise, CCHC is able to work with your funding provider and offer you the care and services you need to improve your health and wellbeing. NDIS Funded

We support clients on the NDIS. Whether plan-managed or self-managed, we offer a range of care and services to support all clients in meeting their goals.

Plan Managed is when the funds allocated by your NDIS plan are administrated by a Registered Provider of Plan Management Services. A Plan Manager helps you to keep track of your NDIS funds and takes care of financial reporting for you.

Self-Managed is when you choose to manage your NDIS funding yourself. This provides you with flexibility and choice on how to spend your NDIS funding to best meet your goals and needs.

CCHC can support your home care needs irrespective of if your NDIS funds are Self-Managed or Plan Managed.

CHSP & HCP Funded

Sometimes approved providers are unable to provide you with the continuity of care you expect or are not able to provide you with the staff you need for your direct care and services. This can be a difficult process to navigate, so at CCHC we ensure you are provided with the right services and care, with continuity and support around the clock. Our team care about the detail, all the points that are important to you. Our team will sit down with you to ensure what is important to you is achieved through your care.

If you are on the CHSP or HCP program and you would like to receive your care and services through CHCC, we can offer a brokerage agreement with your current approved home care provider. Your current provider will take the fees for your care and services out of your approved funding and pay us directly on your behalf.
