Sometimes approved providers are unable to provide you with the continuity of care you expect or are not able to provide you with the staff you need for your direct care and services. This can be a difficult process to navigate, so at CCHC we ensure you are provided with the right services and care, with continuity and support around the clock. Our team care about the detail, all the points that are important to you. Our team will sit down with you to ensure what is important to you is achieved through your care. If you are on the CHSP or HCP program and you would like to receive your care and services through CHCC, we can offer a brokerage agreement with your current approved home care provider. Your current provider will take the fees for your care and services out of your approved funding and pay us directly on your behalf.
