Our team of qualified and knowledgeable case managers/support coordinators take the time to assist you in choosing the most suitable type of care or services for your needs.
Our highly skilled and qualified nurses are available 7 days a week, offering a range of in-home nursing services to meet your individual needs. Whether it’s to manage a health condition or disability, transition home after a hospital stay, or for those choosing to pass away in the comfort of their own home, our experienced team is here to help.
Also available is our team of allied-health professionals, including Physiotherapists, Occupational Therapists, Dietitians, Podiatrists and Social Workers who can visit you in your own home to help with a range of issues affecting your health and well-being.
Activities of Daily Living (ADLs) are the daily essential and routine tasks undertaken to independently care for oneself, such as eating, bathing, and general mobility. For those with a disability, chronic health condition or as we become elderly, some of these tasks become challenging to manage on our own. Here are just some of the things we can help you with:
How you use Client Centred Home Care (CCHC) is up to you. Even if you receive your service delivery from another provider, we will still ensure to monitor and review all the care and services you receive.
If you engage CCHC for Support Coordination or Case Management, it includes all 6 of the following services: Information and Referral, Intake and Assessment, Care Planning, Service Delivery, Ongoing Monitoring of Care and Services, and Ongoing Review of Care and Services.
