From the young to the elderly our team of caring nurses are on hand to support you with your catheter care. We aid clients with SPC and IDC changes, providing information, education and training and supporting you in the prevention and management of common issues that arise from catheterisation.
Constipation and/or faecal incontinence, or a combination of both, can have a negative impact on quality of life. Our team of nurses offer in home, complex bowel care including oral aperients, suppositories, enemas, bowel irrigation, manual removal, digital stimulation, and abdominal massage to assist you with your bowel continence.
Our nurses can offer you in home education and information on managing your stoma and how to avoid complications though techniques, medications, aids, and appliances.
Our clinical nurses have extensive experience in the assessment, treatment, and management of complex wounds. Our team of home care nurses can assess and manage your wounds and skin tears, apply creams and lotions, care for the wound, reduce infection, apply compression and dressings, conduct pressure assessments, and use negative pressure wound therapy to enhance the heeling process.
For those who may be bed bound, in a wheelchair or have limited movement, our nurses will develop a position regime to prevent and reduce the development of pressure ulcers.
Being cared for and spending your last moments in the comfort of your own home is most people’s preference. Our experienced and respectful nursing, allied health and support team work collaboratively to ensure your wishes are carried out at the end stage of your life.
Our nurses and/or social workers can assist you and your family to create an advanced care plan in partnership with your medical practitioner and coordinate the range of health services you may need at home. Our palliative care team is available 24 hours a day – 7 days a week to ensure you receive quality care in the comfort of your own home, round the clock. We utilise our multidisciplinary team to provide holistic and complex nursing care, conduct assessments of your changing needs, control pain and symptoms, prescribe equipment, provide education counselling and support to your loved ones, and work with your GP or other palliative care services.
Once you leave hospital, it is important you continue to get the right healthcare from the right people. Our nursing team can work with you, your doctors, your discharge planner, and your family to ensure you get the best care and support when transitioning from hospital to home.
Our nurses can conduct a cognitive assessment in your own home. Our team are trained in cognitive assessment [‘Rowland Universal Dementia Assessment Scale’ (RUDAS)] which takes around 1 hour to completed. Clients with dementia may be eligible for an additional 10% funding on top of their Home Care Package, depending on the results of this assessment. Our nurses will be able to determine your eligibility for the dementia and cognition supplement.

Early detection of faulty equipment or products can drastically reduce infection prevention and control in most cases.

Our home nursing team can liaise between family GPs and local pharmacists to deliver safe and effective medicine administration. Medication management is an essential service for people who may have cognitive impairment, acquired brain injury, dementia, or an intellectual disability, that need support in scheduling and managing their medications.
Each individual client will be assessed on their abilities to manage their own medications upon intake, that is then annually reviewed (or sooner if there is need for change).
