Supported Living Arrangements
Client Centred Home Care (CCHC) provides Supported Independent Living (SIL) services and supports participants to access suitable Specialist Disability Accommodation (SDA) options throughout South East Queensland.
CCHC supports participants across a broad range of supported living environments – from individual homes and apartments for people requiring dedicated 1:1 support, through to shared residential homes and individual apartment models with CCHC staff onsite 24 hours a day, seven days a week.
We believe finding the right home is about much more than finding an available room. It is about finding the right environment, the right people and the right support team so participants can feel safe, comfortable and
supported to live the life they choose.

Our SIL services are tailored to each participant’s individual needs, preferences and goals, with the level and type of support based on the person rather than a one-size-fits-all model.
